~A PRACTICE NURSE’S INSIGHT INTO HOW YOUR GP PRACTICE CAN SUPPORT YOU~
Guest speaker:
Sally Waldron is a Registered Nurse who has worked in metropolitan and country hospitals and has spent the last 15 years working in GP settings. Her current role is supporting the GP, allied health professionals, patients and their families. Sally has a particular interest in chronic disease prevention and management of the client aged over 65 years.
The role of your GP surgery and ways they can assist you.
· Vaccines: As a community vaccination is important for protecting the more vulnerable
· Care Plan- a plan devised with the nurse, GP and patient for improved management and health outcomes of a chronic disease condition.
· MHCP-a mental health care plan can assist the client with navigating loss, setting goals, boundaries and priorities. A healthy mind is just as important as a healthy body.
· Home Medicine review: Discussing correct storage, timing and method with a pharmacist who visits you at home.
· Advanced Care planning/end of life care: Shared planning for your future health care needs. How to have the conversation with the family and your GP.
